Team Funding Requests
Use these forms to request reimbursement of allowable team expenses, subject to limits established by NSGFA Board. It is preferred (and required for rep teams) that teams submited Ball Toss results prior to submitted a funding request.
Criminal Records Checks
Download and complete the relevant letter below (based on where you live) and take it to your local police detachment, where you will complete and submit a CRC request form. West Van police may charge a fee - get a receipt for reimbursement. Once you receive your CRC, mail a copy (and a copy of your receipt) to: NSGFA President, 1199 Lynn Valley Rd, PO Box 16059 LYNN VALLEY, North Vancouver, BC, V7J 3S9. Keep your original CRC which is valid up to 3 years for any sport.

The above is for pre-planning photo orders. All players to complete and submit a photo form/envelop at the photo booth on Opening Day. Registered players get a Memory Mate (1 individual + 1 team photo) at no extra charge. If you don't want anything else just select Item A1 for $0. Note Bobble Heads do not come with a player photo (you must order or obtain your own photo to insert into the bobble head). The photo schedule is inclued in the above Opening Day Schedule.
Photos are handed out at the NSGFA year-end tournament.
